GoMotion Physio Privacy Policy

This is the privacy policy of GoMotion Physio (registered address 63 Pochard Crescent, Herne Bay, Kent CT65QD). Our privacy policy has been reviewed to incorporate the new rights for individuals set out within the General Data Protection Regulation (GDPR) on 25th May 2018. This major change has occurred to help protect and unify the way that an individual’s data is managed by third parties. 

 

Our privacy policy shows how we promise to look after your personal information as we treat your privacy very seriously.

The information we collect and when we collect it 

This privacy policy relates to our use of any personal information we collect from you in person. Information is collected when you contact the clinic to book an appointment, throughout the assessment and treatment with us, when your personal information changes or is updated and when you submit an enquiry to us via email, telephone, via our website or social media.

Information may include: 

• Personal details such as your name, date of birth, age, address, email address, telephone numbers.  

• Medical information such as your current health, past medical history, prescribed medication and relevant investigation reports and results. 

• Relevant information about your health, lifestyle, weight, sports, hobbies, social circumstances.  

• Information collected from 3rd parties – GP’s, specialists, other health professionals and private medical insurers who we will always seek your permission to contact if necessary.

Personal information we collect automatically 

When you use our website, we automatically receive information on our server logs from your browser or mobile platform, including your location, IP address, cookie information, and the page you requested.

We treat this data as non-personal information, except where we are compelled to do otherwise by law. This is statistical data about our users’ browsing actions and patterns and does not identify any individual. The Google Analytics Terms of Service prohibits the tracking or collection of personal information using Google Analytics, and we adhere to these terms (these can be viewed here: marketingplatform.google.com/about/analytics/terms/us/). If you wish to opt out of being tracked by Google Analytics, we encourage you to look at the Google Analytics Browser Opt-Out Add-on which might serve your needs.

How we use your personal information: the legal basis and purposes  

We will use your data: 

•  To register you as a client at GoMotion Physio and use your details to communicate about appointments and other information about your treatment.  

•  As necessary to help us reach a diagnosis of your presenting problems in order to tailor a treatment programme to you.  

• As necessary for our own legitimate interests, for governance, accounting, managing and auditing our clinic activities to ensure good practice.  

• To disclose your personal data and health information to another health professional, G.P, specialist, medical insurer, school (in the case of a child) or any other relevant bodies or authorities, with your consent.  

• As necessary for compliance with legal and regulatory requirements, related disclosures and establishing and defending of legal rights.  

 

Sharing of your personal data  

Subject to applicable data protection law, your information may be shared:  

• With G.P’s, Consultants, other health professionals. This may be by letter, which is given to you, so the protection of its contents becomes your responsibility.  

• In an emergency or to otherwise protect your vital interests 

• With Legal and/or other professional advisors or courts, to comply with legal requirements.  

• To protect the security and integrity of our clinic practices.  

• We will not provide your personal information to any third parties for the purpose of direct marketing.  

Accessing your personal data

You have the right to access the personal data which we hold on you free of charge and we will provide this information within one month of receipt of request. If the request for data is complex or numerous, we reserve the right to extend this period by a further two months.

Keeping your personal information secure  

We are committed to protecting the confidentiality and security of the information you provide to us. We have in place procedural and electronic processes to safeguard and protect your information.

All paper form information (such as consent forms) are kept securely in locked filing cabinets, only accessed by the practising physiotherapists. All other patient information is held electronically, access to which is password protected. We are subject to a duty of confidentiality and will only process your information in an authorised manner. We provide reasonable security measures in connection with securing personal information held by us, by the following means: 

  • We work to update our security practices to protect your personal data and we review our security procedures carefully; 

  • We comply with appropriate laws and security standards; 

  • We securely transmit your sensitive personal information; 

  • We transmit, protect and access all cardholder information in compliance with the Payment Card Industry’s Data Security standards. 

We have procedures in place to deal with any suspected data security breach. We will notify you of any suspected data security breach where we are legally required to do so.

Data retention  

We have a legal obligation to keep your personal information on file for 8 years after the date of your last attendance. After this time, all patient’s clinical notes held in paper format are securely destroyed. If the record relates to a child or young person, the records must be kept until their 25th birthday.  

Your legal rights 

Under certain circumstances, by law you have the right to: 

  • Request access to your personal data. 

  • Request correction of your personal data. 

  • Request erasure of your personal data. 

  • Object to processing of your personal data. 

  • Request restriction of processing your personal data. 

  • Request transfer of your personal data. 

  • Right to withdraw consent.
